New Patients
CLICK HERE – NEW PATIENT WELCOME KIT – Please return this paperwork to us at least 48 hours in advance of your first appointment including a copy of your medical insurance identification card so your insurance coverage can be confirmed and your chart can be constructed prior to your arrival. Failure to comply with this request will delay your initial visit and may result in that visit being on “Cash Basis” if insurance cannot be validated at that time. Scans can be e-mailed to office@laserdermdoc.com or faxed to us at 714-974-4517.
Monthly Specials And Events
We send our monthly specials and event invitations to you through our interactive communication service that allows you to receive and communicate with us the way you prefer – Text, Email or through our custom app, “Talkwithme”. We assure you that we will never release your e-mail address to any other party nor will we inundate you with emails – just an occasional note so you can stay up to date on new treatment options, treatments specials, products on sale or open house events. This service is fully compliant with patient protection laws and all information you exchange with us through any of the communication methods is held strictly confidential and can only be seen by staff while in the office.
