Businesses must implement all applicable measures listed below, and be prepared to explain why any measure that is not implemented is inapplicable to the business.

Signage:

  • Signage posting a copy of the Re-Opening Protocol at each public entrance of the facility to inform all employees and patients that they should avoid entering the facility if they have a cough or fever; maintain a minimum sixfoot distance from one another; sneeze and cough into a cloth or tissue or, if not available, into one’s elbow; and not shake hands or engage in any unnecessary physical contact.

  • Everyone who can carry out their work duties from home have been directed to do so.
  • All employees have been told not to come to work if sick.
  • Symptom checks, including temperature, are being conducted before employees may enter the facility.

Measures To Prevent Crowds From Gathering (check all that apply to the clinic):

  • Limit the number of patients in the clinic at any one time as outlined in the scheduling section below, which allows for patients and employees to easily maintain at least six-foot distance from one another at all practicable times.

Measures To Keep People At Least Six Feet Apart (check all that apply to the facility):

  • Placing signs outside the clinic reminding people to be at least six feet apart.
  • Placing tape or other markings at least six feet apart at check in and check out locations with signs directing patients to use the markings to maintain distance.
  • All employees have been instructed to maintain at least six feet distance from patients and from each other, except employees may momentarily come closer when necessary to assist with patient exams, accept payment, deliver product, or as otherwise necessary.

  • Disinfecting wipes that are effective against COVID-19 are available to employees at all work stations.
  • Employees to disinfect work areas regularly.
  • Hand sanitizer, soap and water, or effective disinfectant is available near the entrance of the clinic, at checkout counters, and anywhere else inside the clinic where people have direct interactions.
  • Disinfecting all Phreesia pads, payment portals, pens, and styluses after each use.
  • Disinfecting all high-contact surfaces frequently.
  • Remove all reading materials, toys, and any other communal objects from the waiting room and exam rooms.

Measures for Patient Scheduling (check all that apply to the clinic):

  • Assume everyone has been exposed.
  • Ask patients about symptoms during reminder calls. Reschedule the patient appointment if the patient has had a fever, cough, or shortness of breath within 14 days of their scheduled appointment.
  • Remind patients, for their safety and to adhere to social distancing, we are asking that they do not bring family members or friends to their appointment. Minor patients will be allowed to have one parent or legal guardian present throughout the appointment.
  • Patients to wear their own face-mask. If they present to the facility without a face-mask, they will be provided with a cloth reusable face-mask (see attached).
  • Tissues, hand sanitizer, and trash cans are available for patient throughout the facility.
  • Patients will not wait in the waiting room. Allow patients to wait outside or in their car.
  • Take patient temperature and log in the patient chart.
  • Patients will answer the COVID-19 questionnaire and it will be scanned into the patient chart.
  • Reschedule the patient appointment if the patient has a temperature or if the COVID-19 questionnaire answers indicate the patient may have been, or was, exposed to COVID-19.
  • If a patient presents to the clinic with symptoms or the patient becomes symptomatic after being in the clinic, the APM or clinic lead will immediately notify the Chief Operating Officer. The Chief Operating Officer will determine next steps, such as closing the clinic, quarantining employees, etc.
  • For the safety of our patients and employees, to ensure patients are not waiting, and to allow for extra time for cleaning, schedule face-to-face patient appointments as follows:
  • 1-2 providers, schedule 2 patients every 15 minutes;
    3+ providers, schedule 1 patient per provider every 20 minutes.
    *The above scheduling guidelines are a recommendation for re-opening and will be re-evaluated on an on-going basis per clinic.
  • Providers should augment the proposed face to face scheduling recommendations with as many Telehealth visits as medically appropriate.

Measures for Patient Visit (check all that apply to the clinic):

  • All employees to wear scrubs that are washed every day.
  • All employees to wear surgical masks. N95 are only necessary when doing procedures that have patient fumes or spray.
  • Masks to be reused/cleaned every day. Soiled masks will be discarded.
  • All employees to wear eye protection.
  • All employees to clean hands before and after each patient encounter.
  • Employees to open doors for all patients to prevent the patient from touching door handles.
  • Employees will clean all exam rooms and all surfaces touched with disinfectant after each patient visit.
